How can I join the #BFDSmile Loyalty Program?
The annual membership fee for an individual is $399/year and for each additional family member $289/year. Dependent children up to age 22 are eligible. Children may be members only as dependents of adult members.
Can I combine my insurance or discount plan with the loyalty program?
Our dental discount plan cannot be used with other insurance or discount plans. Additionally, it cannot be combined with dental benefits under an automobile medical insurance claim or worker compensation benefits.
When does the #BFDSmiles Loyalty Program begin?
The membership period will begin on the date of payment of either initial payment or renewal and will be valid for one year.
What are your exclusion policies?
- The membership fee must be paid in full at the time of enrollment.
